Photo / Video Release (In-Person)
The undersigned hereby authorizes Golden Heart Academy, or its designee(s) while attending at Golden Heart Academy and/or as a participant in a Golden Heart Academy Program. The undersigned agrees that Golden Heart Academy or its designee may use or permit other persons to use the video and/or visual recordings, negative or prints prepared for such purpose and in such manner as may be deemed necessary, including but not limited to promotional and publicity materials and Golden Heart Academy internet websites, Google, Facebook business account, and other social media platform.
The undersigned hereby waives any right to inspect and/or approve the finished product or products made from such recordings and photographs and waives any claim to royalties or any monetary compensation for appearing in such recordings, photographs, and publications.
This consent form shall be binding on all heirs, successors and assigns and represents the entire agreement between the undersigned and Golden Heart Academy.
